The Forbidden Shrine is located on Iki Island, specifically in the northern part. To be more precise, you can find it northwest of Fort Sakai and north of Sly Hunter's Forest.
You will only be able to access this area if you have completed the Iki Island expansion content or the Director's Cut of Ghost of Tsushima. If you are playing on PC, you already have this content.
