Elden Ring: Shadow of the Erdtree adds three unique forms that you can transform into using reusable items. Omen Form is the odd one out: it's a grotesque, unwieldy transformation that's behind a surprisingly difficult boss. Keep reading to find out how to get Omen Form.

Omen Form is one of three new transformations added in Elden Ring: Shadow of the Erdtree, the other two being Dragon Forms. To obtain Omen Form, you must defeat the boss of Lamenter's Gaol in the northern Charo's Hidden Grave.

This is an optional area that can only be entered by defeating the Dragon's Pit mini-dungeon and boss. You'll find this dungeon in the south of Gravesite Plain, near the Suppressing Pillar. After exiting the dungeon, head south towards the Grand Altar of Dragon Communion and continue on to Charo's Hidden Grave, a high coastal area with red grass that contrasts sharply with the Cerulean Coast. From Charo's Hidden Grave Site of Grace, head northwest around the cliff until you find the entrance to Lamenter's Gaol.
